Hyalite Care Academy’s course work is the result of nearly a decade of expertise, knowledge, and capability associated with the CMS CCM program and other care management programs, which also includes programs such as Principal Care Management (PCM), Behavioral Health Integration (BHI) as well as related programs such as Remote Physiological Monitoring (RPM).

Faith M Jones, MSN, RN, NEA-BC,
Faith Jones is the Director of Care Coordination and Lean Consulting for HealthTech. She currently implements care coordination programs focusing on the Medicare population and teaches concepts related to care coordination and the team-based approach to care across the country. Ms. Jones began her healthcare career in the Navy over 40 years ago. Starting as a hospital corpsman right out of high school, she become an LVN upon discharge from active duty and during her naval reserve time, through the GI Bill, continued on to her ASN from Ventura College in California. She has worked in a variety of roles in clinical practice, education, administration, and consulting. As a working RN, she continued her education to obtain her BSN through the RN-BSN program at the University of Wyoming and eventually completed her MSN in Nursing Administration and Nursing Education at the University of Phoenix. In her leadership roles she has been responsible for the operational leadership for all clinical functions across the healthcare system. She has held a variety of leadership positions in the profession of nursing, including a total of 7-years of service on the American Nurses Association Board of Directors as a Director at Large and the Vice President. She is a past president of the Wyoming Nurses Association and continues to be an active member in WNA/ANA, AAACN, and the Rural Nurses Organization. Her passion for care coordination, team-based care, and advocacy work is apparent in her involvement as a committee member on the AAACN Advocacy Committee, and the AAACN Nurse Sensative Indicator Special Interest Group Advisory Committee. She is a fellow of the American Nurses Advocacy Institute and the ANA-PAC Leadership Society and holds a certification from the ANCC as a Nurse Executive Advanced and is certified in Advance Care Planning and Lean for Healthcare.
